HOME SCHOOL ACHIEVEMENT TEST REGISTRATION

All information you provide during registration is strictly confidential and is not
submitted to any person or agency other than Academic Learning Solutions.

PRE-REGISTRATION IS REQUIRED TO RESERVE YOUR STUDENT'S SPACE.
There are 3 ways to register (listed below).
PAYMENT IS DUE ON THE DAY OF TESTING unless prior arrangement is made.

Once you register, please be considerate and notify Susan if you need to cancel for any reason.
This allows someone on the waiting list to attend on their preferred date.

If for any reason you are late to a session, you will not be permitted to enter the testing room until one of 6 segments of testing is complete. Quietly knock on the door to signal you are waiting, but do not open the door. Susan will come to you within approximately 5 minutes. Any sessions you miss will need to be made up in another test session (for no additional cost) or privately (with additional cost).

REGISTER ONLINE: GO IMMEDIATELY TO THE ONLINE FORM BELOW.
YOU DO NOT NEED TO READ THE TELEPHONE OR EMAIL INSTRUCTIONS.

EMAIL INSTRUCTIONS:
            Send an email to: AcademicLearningSolutions@gmail.com
    Include the following information:
                1. Parent's name.
                2. Phone number and best times to call.
                3. Each student's name
                4. Each student's grade level for testing
                5. Each student's session (date and time)
                6. For each student, state whether or not you want them to have extended time on the test (if needed).

                7. For each student, state whether or not you want them to take the free oral reading screen.
                        The reading screen can detect reading concerns which will not show on the achievement test

                8. State any academic concerns you would like to mention.

                    (I tutor students, home school my own daughter, and hold parent conferences.)
                Include in your message the same items #1-8 listed above for emails.
                    Be sure to speak SLOWLY when leaving your phone number so it is easy to understand
                    (9 & 5 often sound alike as do 1 & 4 at times)
                    Sometimes I return calls in the evening or at night. What time is too late for you?
                    If you would like your confirmation sent by email, include your email address.
                    You should receive a return phone call or email within 48 hours.
                    If not, call again to confirm your registration.

Yes, I want an oral reading assessment for all my students I want an oral reading assessment for some of my students (Please explain in the comment box below) I do not want an oral reading assessment for any of my students I would like more information before I decide about the oral reading assessment I DO want my student(s) to have extended time for sections s/he is unable to finish within the standard time (recommended). See #9 link above for further information. I do NOT want my student(s) to have extended time for the test sections which they are unable to complete during the standard time. (Optional) I have a student who is struggling and/or has other special needs (explain if you want to discuss this further)
